How to ask this girl on a date?

i met her lat august and we were gunna date we both liked each other but we only met twice and she is so pretty but she ened up getting back together with her ex. but now she is single and i actually get nervous talking on facebook because i want her to enjoy talking to me and anyway i want to ask her on a date once she gets back from her vacation what should i say to her cuz i wanna be alone but i get nervous how can i say it and have us just hang out not like going to the movies but just chill and stuff?
